Refer a Young Adult to Anchor Point Living
We partner with DFPS, SSCCs, case managers, probation officers, and child welfare professionals to connect young adults with the housing and support they need to thrive.
Who Can Make a Referral?
Referrals are accepted from authorized child welfare professionals and partner organizations. If you are working with a young adult who may benefit from our Supervised Independent Living program, we encourage you to reach out.

- Single Source Continuum Contractors (SSCCs)
- PAL Specialists
- Probation Officers
- Child Placing Agencies
- Community Partners
Eligibility Criteria
To be considered for placement in our Supervised Independent Living program, a young adult must meet the following criteria:
- Ages 18–22
- Participating in Texas Extended Foster Care
- Referred by DFPS, SSCC, or authorized child welfare partner
- Able to live semi-independently with structured support
- Willing to engage in transition planning and life skills development
The Referral Process

Submit a Referral Inquiry
Complete the referral inquiry form below or contact our team directly. Include as much information as possible about the young adult's needs and current situation.

Initial Review
Our team will review the referral promptly and reach out to discuss eligibility, availability, and next steps — typically within one to two business days.

Assessment & Matching
We conduct an intake assessment to ensure our program is the right fit and to begin developing an individualized transition plan.

Placement & Onboarding
Once approved, we coordinate placement, complete required documentation, and begin the onboarding process with the young adult and their support team.
